The population is aging, and is thus becoming more dependent on the health care system, putting significant demands on the healthcare sector and social services. With the growing shortage in the number of healthcare personnel, elderly patients are likely to be left to self-care or family care.
The most common modality of treating the elderly is the use of medication. Correct medication management is key for the drugs to work. Currently, medication is often not taken as prescribed due to the miscommunication between the doctor, pharmacist and patient. Medication errors also occur as a result of taking medication at the wrong time or dosage. There is a problem here that needs to be addressed.
